MEETING ROOM BOOKING REQUEST FORM
Please complete the form below to request a room booking
Please note:
Booking times must be between 8:30 am and 5:00 pm, Monday through Friday
A minimum of seven business days' notice is required
FWC will confirm your booking within 48 hours of receiving request
Rooms available to book are Simmons Bank Board Room and the Small Conference Room
Only one meeting room may be booked at a time
Meeting room bookings do not include food and parking
